Service Description:

It provides a transitional period of residential care to facilitate ex-mentally ill persons to achieve an optimal level of functioning for the purpose of community reintegration.

Eligibility for the Services:

Ex-mentally ill persons who are:

  • Aged 15 or above, and
  • Physically healthy and free from infectious disease;
  • With reasonable vocational ability and motivation for open employment; and
  • Capable of self-care

Application Procedures:

Referrals can be made by social workers to the Central Referral System for Rehabilitation Services of the Social Welfare Department.

Fees and Charges:

$ 1,171 ($ 842 plus $329) for rent per month.

Service Description:

The Community Mental Health Care Services will include the following services:

  • Brief counseling and development of individual rehabilitation plan;
  • Training in social and communication skills, work habits, budgeting and home management;
  • Arrangement of sheltered workshop/job placement;
  • Referrals to other welfare services

Eligibility for the Service:

  • Ex-mentally ill persons who are discharged from psychiatric hospitals to the community and in need of support services for better reintegration into the community; preferable those firstly discharged and aged 15 to 25;
  • Ex-mentally ill persons discharged from halfway houses; and
  • Ex-mentally ill persons newly admitting to halfway house without a day placement and idling halfway house residents waiting for vocational rehabilitation services.

Application Procedures:

Psychiatrists, social workers and the allied health workers can make direct referral of the ex-mentally ill persons who are discharged from psychiatric wards/hospitals to respective agencies in different regions.

Fee and Charges:

The service is provided free-of-charge.

Service Description:

The Link provides services including:

  • Outreaching visits;
  • Consultation on employment
  • Counseling
  • Networking to local resources
  • Organized social/recreational/educational programs
  • Programs for families/careers
  • Public education programs; and
  • Meals service

Open Hours: Monday to Friday 10 a.m - 6 p.m

Application Procedure:

Referrals can be made:

  • By doctors, social workers, or allied health workers
  • By self-application; or
  • Out-reaching initiated by the Link

Fees and Charges:

The membership fee is $21 for one-off registration
